Description: MCCP provides a novel program model that will provide access to specialty health care services for 70 medically uninsured children who are experiencing serious medical conditions. Care offered is coordinated, continuous, and culturally sensitive, seeking to address chronic conditions at an early stage, thus preventing illnesses that may become more difficult and costly to treat at a later stage. The program includes an interpretation services component for children whose families have limited English proficiency.
