Description: The San Francisco Department of Public Health is the recipient of a FY 2005 Congressional mandate grant award. This grant will fund the Chronically Homeless Offenders Project. The goals of this project are to reduce recidivism in county jails for people with mental health issues; reinforce and extend the medical and behavioral health gains of the participants; provide long-term multi-disciplinary support; and integrate ex-offenders back into the community.
