Description: Faith In Action, in collaboration with area congregations, fire departments, medical personnel, community organizations, hospitals, and community agencies, will provide vital health records, fill these records out, and place them in the designated spot for individuals over the age of 65. By working with pastors and congregations of all faiths, Faith in Action seeks to identify individuals at risk, integrate vital health records throughout all of Faith in Action's programs, and develop critical linkages with community agencies. The goal is to promote health and wellness in persons over the age of 65 so they can maintain their independence and remain in their own homes for as long as possible.
