Description: This proposal objective will focus on the need to effect changes in our medication management process that could yield notable improvements in patient safety, clinical effectiveness, and efficiency. Holy Name Hospital has a medication management redesign that describes a strategy for incorporation and integrating technology that attempts to close the loop for error reduction, ADE (adverse drug events) detection, and integrated data collection in a manner that is unique according to feedback from vendors and literature.
