Description: The technical objective for this proposal is to establishment of an acute pain management consult service at both Conemaugh and WRAMC. This organization will serve as the hospital liaison for pain management issues. As the primary consult service for inpatient pain issues, the organization will be uniquely positioned to perform pain related research that includes the multi-specialty nature of pain management. This will allow for more meaningful research designs that account for the interaction of various pain management approaches. Examples of research questions that would be studied include: What is the role of advanced regional anesthesia (ARA) in civilian and battlefield pain management; What are the costs and cost savings as new pain management technology is introduced into inpatient and outpatient surgery; What is the impact of improved pain control on length of hospital stay, long-term outcome, rehabilitation success, and patient satisfaction; Can improved pain management and ARA improve battlefield casualty efficiency and reduce the medical logistics footprint; and Does ARA and/or other nontraditional pain management techniques impact on chronic pain states? Technical objectives would further establish a regional anesthesia section at Conemaugh, establish a pain management consult service at WRAMC, establish a regional anesthesia fellowship program at WRAMC with rotations at Conemaugh, and determine the role of para-vertebral catheters in the management of post-breast cancer surgery pain.
