Description: Dakota Boys and Girls Ranch's Transition to Community Mentoring Program will provide 50 youth transitioning from residential care to the community with mentors. Identified youth,ages 10 to 18, will receive discharge planning that includes mentors who are matched with mentees and will support the youth in their return to the community. The Dakota Boys and Girls Ranch will train and recruit mentors in the community in which the child is returning. The Transition to Community Mentoring Program will recruit mentors from internal Dakota Boys and Girls Ranch and from community mentoring organizations with outcomes that include outcomes of decreased violence and substance abuse, improved attendance and reduced recidivism. The DGBR will utilize the National Mentoring Mentoring Center's Survey to determine the success of the mentee and mentoring relationships. The intermediate and performance measures will be reported in their categorical progress reports.
